Facility Evaluation Report
On 06/17/2026 at 5:30 AM, Licensing Program Analyst (LPA) Eldin Serrano arrived unannounced at the facility to conduct a Case Management Visit regarding health and safety. This visit was conducted in coordination with Inland Regional Center (IRC). Upon arrival, LPA was greeted by lead staff Caasi Florentino, who was preparing two clients for their scheduled dental appointments. LPA introduced self and explained the purpose of the visit.
During the visit, LPA discussed the reason for the inspection with lead staff Caasi Florentino. A tour of the facility was conducted to assess general health and safety conditions. LPA observed an adequate food supply, including a two-day supply of perishable food and a seven-day supply of nonperishable food and snacks. LPA observed four clients in care and four staff members present. The facility has sufficient staffing to ensure 24-hour a day, 7-day a week coverage.
No deficiencies were observed during this visit. An exit interview was conducted, and a copy of this report was reviewed with and provided to Administrator Francis Pastorfide.
