Complaint Investigation Report

Accommodations were made from 9/1/2022 through 9/3/2022 for the residents in care to ensure that their daily needs were being met.

Based on LPA Allen’s observations, interviews with seven (7) staff members and review of documents The staff took immediate action to ensure that residents daily needs were being met in a comfortable environment.

Based on the investigation, the above finding is Unsubstantiated. A finding of unsubstantiated means although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
