Facility Evaluation Report
Licensing Program Analyst (LPA) Sarina Ramirez conducted an announced visit to confirm that there are no residents at the home for the facility's closure. LPA was granted entrance by Administrator Henry Cachapero
LPA was informed that there are no residents living in the facility. The licensee is initiating this closure. LPA inspected the facility which included the bedrooms, bathrooms, dining area, and kitchen. There were no residents present, and there were no belongings of residents in the facility. Moreover, Licensee/Administrator stated that the reason of the closure is retirement. Licensee surrendered license to LPA.
The effective date of closure will be 08/05/26.
An exit interview was conducted where this report, LIC809 was discussed and provided Administrator Henry Cachapero at the conclusion of the visit.
