Facility Evaluation Report
On 08/07/2026, Licensing Program Analyst (LPA) Sarina Ramirez arrived at the facility unannounced to initiate an investigation for complaint #: 56-AS-20260803091226.
During the visit, LPA toured the facility and observed Resident 1 (R1) washing dishes while one (1) pair of scissors were in the kitchen dish rack. Deficiency will be issued. Licensee immediately locked the scissor during the visit.
Based on observation deficiencies were cited per Title 22, Division 6 of The California Code of Regulations.
An exit interview was conducted where the Licensing reports were discussed and copies of the reports (LIC 809 and LIC 809D) with Appeal Rights was provided to Licensee Mary Swanson at the conclusion of the visit.
