This is an official report of an unannounced visit/investigation of a complaint received in our office on

01/14/2026 and conducted by Evaluator Sarina Ramirez

COMPLAINT CONTROL NUMBER: 56-AS-20260114095648

Licensing Program Analyst (LPA) Sarina Ramirez conducted an unannounced visit to deliver findings on the allegation mentioned. LPA met with Administrator Leticia Cerdenio and explained the purpose of the visit.

Regarding the allegation, LPA conducted Interviews with the Administrator, staff, and outside parties. LPA reviewed R1’s facility file documents, and the death report. There was no evidence to corroborate the allegation.

Therefore, the above allegation is UNSUBSTANTIATED. A finding of unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

An exit interview was conducted where this report was discussed and provided to Administrtaor at the conclusion of the visit.
