Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced case management visit to the facility to follow-up on an incident report received by the regional office on 4/23/24. LPA Malcore met with Suzan Abou-Hebeish, Administrator, and informed the purpose of the visit.

During today’s visit, LPA obtained documents pertinent to the incident and conducted a health and safety check. No immediate health and safety concerns were observed. LPA informed the Administrator that, at this time, the incident requires further review by the Department.

An exit interview was conducted, and a copy of this report was provided to the Administrator at the conclusion of the visit.
