On 04/08/2026 at 10:00AM Licensing Program Analysts (LPAs) Renese Howell-Small and Becky Mann arrived unannounced to the facility in order to conduct a collateral visit regarding complaint # 56-AS-20260401160806.
LPAs observed one (1) client during the visit, conducted an interview with staff and contacted the Administrator, Vivian Francisco to inform her of the purpose of the visit.
An exit interview was conducted, where this report LIC809 was discussed and a copy was provided to staff, Shonte' Slaughter.
