The inspector’s account

On 10/17/2025 Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a Case Management visit on an uncleared adult at the facility.

During the facility's annual inspection, LPA observed Staff 1 (S1) arrived at the facility and entered the front side gate into the backyard. S1 knocked on the screen door, obtained a key from the Administrator and stated that they were going to add a chemical into the swimming pool.

On 04/11/2025 a deficiency was cited regarding S1 for not obtaining a criminal record clearance.

On 05/21/2025 Licensee George Clardy attended an office meeting with the Department and discussed client's behavioral plan, hiring of staff and criminal record clearance.

As of 10/17/2025 LPA could not confirm S1 has obtained a criminal record clearance. A deficiency will be cited and a civil penalty will be assessed.

An exit interview was conducted where this report LIC809, LIC809D, LIC421IM and Appeal Rights were discussed and a copy provided to Administrator, Vivian Francisco.
