Staff engaged in inappropriate interactions with client
Staff made inappropriate comments towards client
On 03/18/2025 at 2:55PM Licensing Program Analyst (LPA) Renese Howell-Small conducted an unannounced visit to the facility in order to deliver findings for the above allegations. LPA discussed the purpose of the visit with Licensee, George Clardy. The investigation consisted of interviews and record review.
In regards to the allegation of staff engaged in inappropriate interactions with client:
LPA interviewed seven (7) staff and one (1) client. Staff denied witnessing other staff engage in inappropriate interactions with clients or chasing clients out of the facility. Client 1 (C1) attempts to elope from the facility as part of C1's behavior. Staff are trained to follow C1 out of the facility to ensure C1's safety and to redirect behavior. Therefore, this allegation is UNSUBSTANTIATED.
In regards to the allegation of staff made inappropriate comments towards client:
LPA interviewed seven (7) staff and one (1) client. Client 2 (C2) stated that staff are nice and denied hearing a staff say they "hate" clients. All staff denied that staff made inappropriate comments towards clients. Staff also denied witnessing other staff state "they hate" clients in care. Staff treat clients with respect and care. Therefore, this allegation is UNSUBSTANTIATED.
UNSUBSTANTIATED is defined as the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted where this report LIC9099 was discussed and a copy was given to Licensee, George Clardy.
