Complaint Investigation Report
Interviews were conducted with staff. Staff 6 (S6) denied that sexual abuse occurred. Interview with (S6) revealed that S6 assists with preparing meals and cleaning while other staff assist residents with showering. Interview with (S1) revealed they have not witnessed or observed any sexual misconduct by staff. All staff interviewed denied the allegation. On 06/06/2025 an interview was conducted with staff at the Adult Day Program where R2 and R3 attends. Staff interviewed denied observing signs of sexual abuse and stated the residents’ behavior are consistent. Based on observations, interviews, and a review of facility records, there is insufficient evidence to support the allegation. Therefore, this allegation is UNSUBSTANTIATED.
An UNSUBSTANTIATED complaint is defined as the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted where this report LIC9099 and LIC9099C were discussed and a copy provided to staff, Zorina Francisco.
