Based on the information obtained during the investigation, this agency has substantiated the complaint allegation as valid and that a violation has occurred based on the preponderance of available evidence. An exit interview was conducted and a copy of this report, LIC9099-D, and appeal rights were provided to Executive director Marguerite Crockem.
