Facility Evaluation Report
Licensing Program Analysts (LPAs) Magda Malcore and Eldin Serrano made an unannounced visit to the facility. The purpose of the visit was to conduct a post licensing inspection. LPAs met with House Manager, Erika Vargas, informed the purpose of the visit and were granted entry to the facility.
The facility is a (4) client bedroom, one (1) staff room, (4) bathrooms home with a kitchen/dining area, living room/activity room, office area and attached garage. The facility is an Adult Residential Facility (ARF) with a licensed capacity of four (4) and a current census of four (4) clients. LPAs conduct a overall inspection, which included, but was not limited to, the following:
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). There are no obstructions to indoor and outdoor passageways. Outdoor activity space is shaded and enclosed with a self latching gate. The facility is maintained at a comfortable temperature of 72 degrees Fahrenheit. Client bedrooms were equipped with mattresses, nightstands, storage space, and lighting. Client bathrooms were clean, and appliances were operating appropriately. The hot water temperature in client central bathroom (located near laundry room) measured 130 degrees Fahrenheit. The House Manager stated that a maintenance person will come and adjust the water temperature by next day. The facility is equipped with smoke detectors and carbon monoxide alarms, three (3) fully charged fire extinguishers, laundry equipment, covered fireplace, first aid kit w/manual, telephone service, emergency supplies and water. The facility has posted in a common area: client personal rights, the CCLD complaint poster, emergency telephone numbers, activities calendar, menu and facility license.
Care & Supervision: The facility has sufficient number of staff to provide care and supervision to the clients.
