Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA was granted entry and met with Licensee, Christopher Clark. The facility is a certified Inland Regional Center (IRC) vendor with a license capacity of (3) clients and a current census of (0) clients.
During today's visit, Licensee Clark and Licensee Valencia Johnson (via telephone) stated that they no longer have interest in maintaining their license. The reason given was due to Inland Regional Center not providing them with clients. The facility is currently being rented out to two (2) private parties and the Licensees are not providing care and supervision to the private parties.
LPA informed Licensees to surrender their license and submit a letter of license forfeiture to Community Care Licensing Division's Regional office. Licensee has been informed that the would need to reapply for a new licensee once facility has been closed and that no care and supervision is allowed without a valid or current license.
An exit interview was conducted where this report was discussed and a copy was provided to Licensee Clark at the conclusion of the visit.
