Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced visit to the facility to discuss an incident report that was received by the Department on 06/11/25. LPA met with Staff, Star Pillman.
During today's visit, it was discovered that client 1 (C1) has a Restricted Health Condition Care Plan in place where staff 1 (S1) was not trained by a licensed professional and was testing C1's glucose.
One deficiency was cited during today's visit. An exit interview was conducted where a copy of this report LIC809, LIC809D and appeal rights were provided to Staff, Star Pillman.
