Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to a Special Incident Report (SIR) submitted to the Community Care Licensing Office on 08/22/25. LPA was greeted and met by HR Assistant, Jessica Zaragoza at the reception area. LPA introduced self and stated purpose of the visit. Administrator, Jasmine Weber later walked in and LPA introduced self and stated the purpose of the visit.
On 08/20/25, R1 attempted to sexual abuse R2 without R2's consent.
During today's visit, LPA met with the administrator to discuss the incident and surrounding events. LPA conducted an interview and record review. During interview, it was discovered that R1 was arrested by law enforcement and taken into custody. Administrator stated that the facility has initiated a plan to prevent future occurrences.
No deficiencies were cited during today's visit. An exit interview was conducted where a copy of this report was provided to Administrator, Jasmine Weber.
