Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to conduct an annual inspection of the facility. LPA was granted entry and met with Licensee Janice Goodrich.

During today's visit, Licensee informed LPA that on 07/02/2025, she surrendered the facility license via mail to the Department of Social Services mailing address. The Licensee no longer seeks to continue facility operations due to retirement. The Licensee has not had residents or staff since October 2024. LPA observed no residents in care.

The effective date of closure will be 07/11/2025.

An exit interview was conducted where this report discussed and a copy provided to Licensee Goodrich at the conclusion of the visit.
