Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Program Director (PD) Amanda Glaspie and discussed the purpose of the visit. The facility is an Adult Day Program (ADP) with a license capacity of (75) with (28) clients in attendance today and (13) staff working. LPA conducted an inspection, which included, but was not limited to, the following:
Operation/Physical Plant: The facility's indoor and outdoor passageways were kept free of obstruction. The facility has sufficient activity space and supplies for client activities. Activity areas include a computer/lab area, Gym area, Art room, Garden room, Salon/Locker room. The temperature in facility rooms measured 72 and 75 degrees Fahrenheit (F). Client bathrooms were maintained clean and odor free. The hot water temperature in client bathrooms measured 106 and 112 degrees (F). The facility is equipped with carbon monoxide alarms, fire extinguishers, and emergency lighting. Cleaning supplies, disinfectants and other dangerous items were kept locked. Client medications are centrally stored in a locked cabinet and the facility maintains a first aid kit with manual. The facility has posted facility license, oxygen in use signs, Community Care Licensing Complaint poster, evacuation sketch, emergency telephone numbers, and client personal rights.
Food Service: Program provides snacks to clients. The facility has a sufficient supply of bottled water for clients. **continued on LIC809-C**
