Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced case management visit to the facility. The purpose of the visit is to follow up on an incident report submitted by the facility to Community Care Licensing Division's Regional office. LPA introduced self and discussed the purpose of the visit with Program Director, Simien Hires.

The facility reported that on December 19, 2025, Staff #1 (S1) spoke inappropriately to Client #1 (C1) during a community outing. During today's visit, the LPA reviewed pertinent documents and interviewed staff and clients regarding the incident.

The case management has been concluded, and no deficiencies were cited related to the above incident.

An exit interview was conducted, during which this report was discussed, and a copy was provided to Program Director Hires at the conclusion of the visit.
