Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Program Director (PD) Simien Hires and discussed the purpose of the visit. The facility is an Adult Day Program (ADP) with a license capacity of (75) and a current census of (24). LPA conducted an overall inspection, which included, but was not limited to, the following:

Operation/Physical Plant: The facility has a sufficient care staff to assist clients at program. The facility's passageways were kept free of obstruction. The facility has sufficient activity space and supplies for client use. The facility is maintained at a comfortable temperature of 73 degrees F. Client bathrooms were maintained clean and odor free. The hot water temperature in client bathrooms tested at 106 degrees F. The facility is equipped with carbon monoxide alarms, fire extinguishers, first aid supplies, and emergency lighting. Cleaning supplies, disinfectants and other dangerous items were kept locked. Emergency disaster plan and drills are current.

Food Service: Program provides snacks to clients. The facility has a sufficient supply of bottled water for clients. LPA observed that the ceiling vents in the kitchen area were surrounded with dark dust. The PD stated that the vents will be clean by end of day.

Record Review: Staff records were reviewed First Aid and CPI certifications, criminal record clearances, job training, and health screenings. Client records were reviewed for admissions agreements, medical assessments, and needs and service plans.

During today's visit, a deficiency was cited in accordance with Title 22 of the California Code of Regulations (CCR). An exit interview was conducted where this report was discussed and copy of this report was provided with appeal rights to the (PD) at the conclusion of the visit.
