Facility Evaluation Report

Licensing Program Analyst (LPA) Michelle Echeverria made an unannounced Collateral visit to interview one client in regards to complaint investigation (Complaint Control #56-AS-20260721110256). Upon arrival, LPA met with Amanda Galaspie, Program Director, introduce self and stated the purpose of the visit.

An exit interview was conducted and a copy of this report was provided to the Program Director, Amanda Galaspie.
