The inspector’s account

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced required Annual Inspection. LPA met with Program Case Manager, Tammy Higgins who was advised of the purpose of the visit. The facility is currently licensed as an Adult Day Program and has 70 clients present. With the assistance of Lead Program Aide, Clara Escamilla, LPA conduct an overall inspection of the facility, which included, but was not limited to, the following:

Facility passageways are clear of obstructions. Client activity rooms were equipped with required furniture and activity supplies/equipment for clients. Client bathrooms were observed to be in sanitary conditions and equipped with grab bars. LPA observed that the hot water in two client bathrooms located near the laundry room (Wing A) attained a temperature less than 105 degrees Fahrenheit(F). The water temperature in the two client bathrooms tested at 63 degrees F. Disinfectants, toxins, sharps and cleaning supplies are locked and inaccessible to clients. There are no firearms/ammunition or bodies of water. LPA observed the kitchen area to be clean, free of odors, and maintained in a healthful manner. LPA observed a sufficient amount of snacks, bottled water, emergency food, and personal hygiene products for clients. Medication is locked and inaccessible to clients in the facility. The facility is equipped with fire extinguishers, smoke alarms, carbon monoxide alarms, and first aid kit. The last earthquake drill was conducted on 12/16/2025. LPA reviewed the Infection Control Plan and Annual Fire Inspection Report.

LPA reviewed four (4) client files for admission agreements, updated physician reports, and needs and services plans. LPA reviewed four (4) staff files for First Aid/CPR certifications, criminal record clearances, orientations/training, and health screenings.

A deficiency was cited per Title 22, Division 6, of the California Code of Regulations (CCR). An exit interview was conducted, reports LIC809/LIC809D were discussed and provided with appeal rights to Case Manager, Higgins.
