Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator/Licensee Tiyesha Rias, and discussed the purpose of the visit.
The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6) non-ambulatory residents and a current census of (0) residents in care. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following:
Physical Plant/Environment: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). Indoor and outdoor passageways are free of obstruction. The facility has no swimming pools or similar bodies of water. Indoor activity areas were maintained clean and sufficient for residents. Outdoor shaded area is fenced and sufficient for resident activities. The facility is maintained at a temperature of 73 degrees Fahrenheit. Resident two (2) bathrooms were equipped with slip resistant material in showers and grab rails. The hot water temperature in residents' bathrooms measured 106 degrees Fahrenheit. Resident three (3) bedrooms had sufficient lighting and furniture in good repair. Facility is equipped with carbon monoxide alarms/fire alarms, first aid kit, and fire extinguisher, CCLD complaint poster, facility sketch, resident council rights, theft & loss policy. Sharps, disinfectants, and cleaning solutions were kept inaccessible to residents. The facility maintains a locked cabinet where resident medications will be stored.
Food Service: Facility kitchen and dining areas were maintained clean. The facility has sufficient storage areas for non-perishable and perishable foods.
Personnel/Resident Records: There are no residents and no staff working at the facility.
No deficiencies were cited during today’s visit. An exit interview was conducted. A copy of this report was provided to the Administrator/Licensee.
