Facility Evaluation Report

Licensing Program Analyst (LPA) LaVette Farlow arrived at the facility unannounced to conduct a Case Management Visit and to follow up on complaint control number 56-AS-20231127133521, and conduct interviews. LPA was greeted by Front desk security, Jonathan Jaime. LPA asked if the Executive Director was available to meet with LPA to discuss the purpose of the visit and to notify her of my presents. LPA introduced self and stated purpose of the visit. The Executive Director Tammy Chavez escorted LPA to an office space to work. LPA Farlow was escorted on a tour of the facility and conducted interviews of residents in care.

During today's visit, LPA discussed the complaint with Tammy Chavez, Executive Director, and Raj Thandi, Chief Compliance Officer who had questions regarding the visit over the phone. LPA interviewed residents, interviewed staff, requested and received documents pertaining to the investigation.

No deficiencies were observed during this visit. An exit interview was conducted where this report was, reviewed, discussed and then provided to Tammy Chavez, Executive Director.
