The inspector’s account

On 02/19/2026, at 12:15 PM, Licensing Program Analyst (LPA) Beena Singh made an unannounced visit to the facility. The purpose of the visit was to conduct a health and safety visit as facility accepts its first resident for placement following its initial licensor. LPA Singh contacted the licensee via phone call who arrived at the facility after 15-20 minutes, LPA Singh met with Licensee Satinderpal Singh Rai and was granted entry to the facility. Licensee informed LPA Singh that there is one(1) client admitted to the facility who was at the day-program during the visit and arrived during this visit.

This inspection has done within 90 days after a facility accepts its first resident for placement on 2/2/2026, following its initial licensor, LPA Singh has inspected the facility, walk through has been conducted, to evaluate facility/Licensee compliance with rules and regulations and to assess the facility's continuing ability to meet regulatory requirements.

LPA Singh and Staff Funke Omole completed a walk through of the facility. Licensee informed LPA Singh that there is one client who is out in the community-day-program. Bedrooms and bathrooms were clean, temperature was maintained within regulations. Posters of licensing were observed posted throughout facility, LPA Singh did a walk through of the facility inside and outside perimeter, no health and safety concerns observed. Facility is clean and sanitary for the clients in care.
