Facility Evaluation Report
On 10/02/2025 at 12:30 PM, Licensing Program Analyst (LPA) Beena Singh conducted an unannounced visit to the facility to conduct the required comprehensive annual inspection. LPA Singh greeted by Staff- House Lead Darryl Marks and gained access at the home. LPA explained the purpose of the visit to House Lead Darryl Marks.
The facility has four (4) bedrooms, three (3) bathrooms, kitchen, dining room, living room, attached garage, and backyard. The facility is vendorized by Inland Regional Center (IRC). LPA Singh completed a walk through of the facility, review of records and medications audit.
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD), LPA observed one(1) client during the visit as facility staff-House Lead Darryl Marks reported to LPA that Client #2 (C2) and client#3 (C3) are out in the community. There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 75 degrees Fahrenheit. LPA inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs, and sufficient lighting. LPA Singh inspected client bathrooms; bathrooms were clean, and appliances were found functional. Water temperatures tested at 77 degrees Fahrenheit. Deficiency will be issued. The facility is equipped with operational combined smoke detectors and carbon monoxide detectors, charged fire extinguishers, and first aid kit with first aid book. In addition, LPA observed non-slip mat in clients bathroom.
Posters such as the personal rights, CCLD complaint poster, and emergency disaster plan were posted in a common area. Client medications were kept in secure cabinets inaccessible to clients. LPA Singh observed night lights at the hallway leading to clients' shared bathrooms. The facility had emergency kits, emergency food and water.
