Facility Evaluation Report
On 05/27/2026, Licensing Program Analyst (LPA) Beena Singh Licensing Program Analyst (LPA) Beena Singh made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA Singh called Licensee Constance Odudu and arrived in 45 minutes. An initial application to operate an Adult Residential Facility (ARF) was submitted to the Central Applications Bureau (CAB) on 8/16/2024, for a total capacity of four (4), three(3) Ambulatory and one(1) non-Ambulatory.
Fire clearance was granted on 10/24/2024. LPA met with Facility Licensee/Administrator and was granted entry to the facility. At the time of the visit there were no staff and no clients present. The facility is a five (5) bedroom, three (3), bathroom home, with a kitchen/dining area, living room, with a detached garage. The facility is a Residential Care Facility for Elderly (RCFE) Licensed capacity is (6) current census (6). LPA was accompanied by Facility Licensee, to conduct a general overall inspection, which included, but was not limited to, the following:
Physical Plant: The Facility is a two (2) story, five (5) bedrooms house, with four (4) client bedrooms, one (1) staff bedroom and three (3) full bathrooms, living room, dining area and kitchen. There was an attached two (2) car garage in the left side of the house . The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected residents’ bedrooms; they are equipped with required furniture such as mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperatures within regulations.
