Facility Evaluation Report
On 4/11/2025 at 8:45 AM, Licensing Program Analyst (LPA) Beena Singh conducted an announced visit to the facility for purpose of Pre-licensing evaluation. LPA met with Administrator Constance Odudu.. An initial application to operate an Adult Residential Facility (ARF) was submitted to the Central Applications Bureau (CAB) on 8/16/2024, for a total capacity of four (4), three(3) Ambulatory and one(1) non-Ambulatory.
Fire clearance was granted on 10/24/2024. LPA Singh observed the following:
Facility was a two (2) story, five (5) bedrooms house, with four (4) client bedrooms, one (1) staff bedroom and three (3) full bathrooms, living room, dining area and kitchen. There was an attached two (2) car garage in the left side of the house.

Each client bedrooms accommodate any ambulatory client. All client bedrooms were adequately furnished.
with bed, chair, closet, appropriate linens, adequate lighting, a lamp and an operable smoke/carbon monoxide alarm.
The two(2) full bathrooms for clients and one staff bathroom, have a working toilet, wash basin, and shower with an adequate supply of toilet paper and soap.
