Facility Evaluation Report

On 05/30/2025 at 11:19 AM Licensing Program Analyst (LPA) Beena Singh conducted an unannounced inspection in order to increase the capacity per licensee request. LPA Beena Singh met with Licensee/ Administrators George Garcia and Jeanette Garcia and toured the facility.

Per the LIC200, Licensee requested for the capacity increase from two (2) Ambulatory to three (3) ambulatory. The fire clearance request was approved on 05/05/2025 for three (3) Ambulatory clients.

Licensee request for capacity increase approval of an additional bedroom for an increase of capacity by one (1), for a new total of three (3) clients. Fire Clearance is approved for three (3) ambulatory residents on 05/5/2025 by the Fontana Fire Department. LPA Singh observed one client was at the facility and Licensee/Administrator George Garcia informed LPA Singh that one client was out in the community.

LPA toured the facility and observed the client bedroom to be in sufficient compliance to grant the capacity increase. The facility currently has three (3) bedrooms, two ½ bathrooms, one kitchen, and one living room/dining room, backyard and a two car garage. LPA observed the bedroom to have the necessary equipment such as the required bed, mattress, linen, chair, lamp, dresser and storage, an operable smoke detector and has sufficient supplies for the proposed capacity increase.
