Facility Evaluation Report

On 10/24/2025 at 1:00 PM, Licensing Program Analyst (LPA) Beena Singh arrived at the first step independent living-Day Program unannounced for a collateral visit on a complaint investigation to interview Client#1, regarding complaint control number 56-AS-20251009140110 .

The facility is an Adult Day Program (ADP) Licensed capacity is (60) current census (42). During this visit LPA Singh was met by Day program Director, Natalie Boubion and Day Program supervisor- Edward Vazquez.

LPA Singh met with Client#1s family and interviewed them at the day program for the ongoing investigation.

An exit interview was conducted, and a copy of this report (LIC809) was discussed and provided to Day Program supervisor-Edward Vazquez
