Facility Evaluation Report
On 03/13/2026 at 11:30 Licensing Program Analyst (LPA) Beena Singh conducted an unannounced visit to the facility to conduct the required comprehensive annual inspection. LPA Beena Singh was greeted and granted entry by Staff Anita Gregorio. LPA Beena Singh explained the purpose of today's visit to Staff Anita Gregorio. Current census is two(2) clients and one(1) was present at the facility during the visit and other client was out in the community.
The facility has four (4) bedrooms, two (2) bathrooms, kitchen, dining room, living room, attached garage, and backyard. The facility is vendorized by Inland Regional Center. LPA Singh completed a walk-through of the facility, review of records, P and I audit of Client#1, Client#2 does not have P&I. LPA Singh completed medications audit, no issues.
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD), LPA Singh observed one client during the visit. There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 70 degrees Fahrenheit. LPA Singh inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs, and sufficient lighting. LPA Singh inspected client bathrooms; bathrooms were clean, and appliances were found functional and water temperatures tested at 115 degrees Fahrenheit. The facility is equipped with operational smoke detectors, carbon monoxide detectors, charged fire extinguisher, and first aid kit with first aid book.
