Facility Evaluation Report

On, Licensing Program Analyst (LPA) Beena Singh arrived unannounced to conduct the required comprehensive annual inspection to the facility. LPA Singh met Staff #1 (S1) Barbara R. Guiterrez., introduced self and stated the purpose of the visit.

The facility has four (4) bedrooms, three (3) bathrooms, kitchen, dining room, living room, and attached garage. The facility is vendorized by Inland Regional Center (IRC). LPA Singh completed a walk through of the facility, review of records, and medication audit.

Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD), LPA Singh observed no clients, client#1 was at the day program. There are no obstructions to the indoor and outdoor passageway of the facility. The facility is maintained at a comfortable temperature of 75 degrees Fahrenheit (F). LPA inspected client bedrooms; they are equipped with required furniture such as mattresses, storage space, chairs, night-stands and sufficient lighting. LPA inspected client bathrooms; bathrooms were clean, and appliances were found functional. The water temperature tested at 115 degrees Fahrenheit.

The facility is equipped with operational smoke detectors, carbon monoxide detectors, charged fire extinguisher, and first aid kit with a first aid book. LPA observed night lights maintained in hallways and passages to non-private bathrooms.
