Facility Evaluation Report

Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to conduct a complaint of investigation related to complaint (56-AS-20251007154245 ). During this visit the facility was cited to care and supervision violations. The deficiencies were discussed with Administrator Acabal and agreed that there was a lack of care and supervision by staff, as well as Administrator hours should be increase. On this day, a deficiency was cited for Administrator hours. This report was signed by LPA Prieto and Administrator Acabal and a copy of this report, as well as the LIC 809D, and a copy was left with the facility.
