Facility Evaluation Report
Licensing Program Analyst (LPA) Edith Conchas made an unannounced visit to the facility to conduct a required comprehensive annual inspection. LPA met with Ivy Cervania, DSP who assisted with the tour of the facility.
The facility is an Adult Residential Facility (ARF) with license capacity of six (6) and a current census of six (6). The facility is vendored through Inland Regional Center (IRC). LPA conducted an overall inspection of the facility where it is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected residents’ bedrooms in which they are equipped with required furniture such as mattresses, night stands, storage space, and sufficient lighting. The bathrooms were clean, and most appliances were operated appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors, fully charged fire extinguishers, and carbon monoxide detectors. Posters such as personal rights, the CCL complaint poster, and the disaster plan, infection control plan, facility license, facility sketch, menu and activities were posted in a common area. LPA observed there was a designated storage space for resident/staff files. Medications are kept locked in the kitchen cabinet inaccessible to residents in care. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. Non-perishable and perishable food supply is sufficient for the number of residents in care. Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. LPA reviewed client files for admission agreements, updated physician reports, and needs and services plans which appeared to be current. Medications were also audited. LPA reviewed staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings and were complete and current with the required documentation. LPA observed liability and car insurance be current.
