Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced required 1-year visit to the facility. LPA met with Cielita Ravelo, Facility Representative and discussed the purpose of the visit. The facility is an Adult Residential Facility (ARF) with license capacity of (4) and a current census of (0). The facility is in process of being vendorized through Inland Regional Center (IRC). The facility currently has no staff and no clients in care. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following:
LPA observed indoor and outdoor passageways are kept free of obstruction. The fenced backyard has sufficient shaded space for outdoor activities. No bodies of water were observed. The facility has sufficient lighting and is maintained at a comfortable temperature. LPA inspected the kitchen. Hot water temperature tested at 109 degrees F. Facility has sufficient nonperishable and perishable storage area. Freezers and refrigerators are maintained in a safe and healthful manner. Facility has sufficient utensils, cups and plates. Menu sample posted shows a variety of nutritional meals. Sharps, disinfectants, cleaning supplies are kept safe and locked. Facility has a sufficient supply of towels, linen, and personal hygiene products for clients. LPA inspected client bedrooms. Bedrooms are equipped with beds in good repair, bed linens, nightstands, and sufficient lighting. LPA inspected client bathrooms. Bathrooms are operating in a safe and sanitary condition. The hot water temperatures tested between 105 and 108 degrees F. The facility has operating carbon monoxide alarms. LPA observed personal rights, emergency disaster plan and telephone numbers, Community Care Licensing complaint poster, posted in a common area. LPA observed the cabinet where medications will be stored, which is kept safe and locked. No deficiencies were cited during today's visit. An exit interview was conducted where this report was discussed, and a copy of this report was provided to the facility representative at the conclusion of the visit.
