Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit to the facility. LPA met with Senior Business Office Director (SBOD), Chelsea Plank, and Community Sales Director (CSD), Margo Guertin, informed the purpose for the visit.
During today's visit, LPA conducted a tour of the facility and interviews pertaining to complaint investigations 56-AS-20260430081726 & 56-AS-20260415121729 .
An exit interview was conducted and a copy of this report was provided to CSD Guertin at the conclusion of the visit.
