Facility Evaluation Report
Licensing Program Analyst (LPA) Becky Mann visited the facility unannounced for a case management visit. This case management visit is in response to a Special Incident Report (SIR) submitted to Community Care Licensing (CCLD) received from the facility. LPA met with Assistant Administrator Jonetta Williams and explained the purpose of the visit.
The SIR documented that on September 07, 2025, Client #1 (C1) stated that they consumed the lotion and was told by Staff #1 (S1) "go ahead, you're just going to throw it up." C1 attempted to consume nail polish but was removed by S1. C1 stated that they consumed alcohol wipes but the taste from the alcohol was intense. C1 spit it out.
During today's visit, LPA discussed the incident and the surrounding events. LPA interviewed staff, clients and conducted a record review.
No health and safety concerns were discovered, and the investigation into this incident is closed.
No deficiencies were cited during this visit. An exit interview was conducted where this report (LIC809) was discussed and a copy provided to Assistant Administrator, Jonetta Williams
