Facility Evaluation Report
Licensing Program Analyst (LPA) Becky Mann conducted an unannounced case management visit to follow up on a Special Incident Report (SIR) sent to licensing dated on 2/27/2026. LPA was greeted and explained the reason for the visit to Kory Medina, Assistant Administrator .
Per the incident report, Crisis Prevention Intervention (CPI) was utilized on a client who was in behavior. LPA reviewed facility records and observed that staff conducted debriefing within 24 hours according to Title 22, California Code of Regulations 85168.3.
No deficiency was cited during this visit. An exit interview was conducted where this report LIC809 was discussed and provided to Kory Medina, Assistant Administrator .
