Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit to the facility. During today's visits, LPA met with Administrator, Davonna Mason and House Lead, Tashari Harris and was granted entry into the facility.
At 1:30 p.m LPA arrived at the facility and informed the Administrator the purpose of the visit was to interview clients regarding open complaint investigation. LPA was informed by Administrator that the there were no clients current at the facility. Two (2) clients were at day program and one (1) client was out in the community with a family member. LPA requested day program information from the Administrator for the two (2) clients. LPA was informed by the day program director that client #1 (C1) did not attend program today and was at the home sick. At around 2:20 p.m, LPA contacted facility Administrator Mason via telephone to inquire whereabouts of C1. Administrator stated that C1 was actually at the facility and did not attend program because they were sick. Administrator stated they were misinformed by staff. LPA returned to the facility to ensure C1 was at the facility and conduct interviews.
Deficiencies were cited during today's visit per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted and a copy of this report with appeal rights was provided to House Lead, Harris.
