Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA was granted entry, met with Staff, Erlinda Spears, and informed the purpose of the visit. Administrator, Leodigaerl "Glenn" Panopio arrived later to the facility. The facility is an Adult Residential facility with a license capacity of (5) and a current census (3). The facility is a certified Inland Regional Center (IRC) vendor. During todays visit, all three clients were attending Day program. LPA conducted an overall inspection, which included, but was not limited to, the following:
Operation/Physical Plant: Indoor and outdoor passageways were kept free of obstruction. The facility has no swimming pools or similar bodies of water. The facility has sufficient indoor and outdoor activity space for clients in care. The facility is maintained at comfortable temperature of 74 degrees Fahrenheit. Outdoor activity space is shaded and enclosed with latching gates. The facility is equipped with operating smoke/carbon monoxide alarms, fully charged fire extinguisher, laundry equipment, covered fireplace, hallway lighting, activities supplies, emergency supplies/water, and telephone service. Client bedrooms were equipped with beds, bed linen, chairs, storage space and lighting. Client bathroom equipment were fully operating and showers were equipped with non-skid material. The hot water in client bathrooms tested at 109 degrees F. LPA observed a bottle marked as clorox disinfectant stored unlocked and unattended in the hallway bathroom of the facility. Staff present stated that had been cleaning earlier and left the bottle under the sink. The facility has posted in a common area: Community Care Licensing complaint poster, Ombudsman poster, activities, facility license, disaster plan, emergency telephone numbers, client personal rights, and facility sketch. The Administrator was informed that annual licensing fees are due April 24, 2026.
Food Service: The facility’s dining areas and kitchen are maintained clean. The facility has sufficient non-perishable and perishable food for number of clients in care. The facility has sufficient cups, plates, and utensils for client use.**continued on LIC9099C**
