Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore conducted an announced visit to confirm that there are no clients at the home for the facility's closure. LPA met with and was granted entrance by Licensee/Administrator Donna Nazir.

LPA conducted a tour of the facility and observed no clients present or client belongings in the facility. The property is currently being utilized as a private family home. Licensee/Administrator Nazir stated they surrendered their License via mail to Community Care Licensing around 08/01/2025. The reason for closure is due to lack of clients to continue facility operation.

The effective date of closure will be 08/11/2025.

An exit interview was conducted where this report was discussed and a copy provided to Licensee/Administrator Nazir at the conclusion of the visit.
