Facility Evaluation Report

Licensing Program Analysts (LPAs) Magda Malcore and Eldin Serrano made an unannounced visit to the facility to conduct a required annual inspection. LPAs met with Kara Mitchell, Case Manager,and discussed the purpose of the visit. The facility is an Adult Day Program (ADP) with a license capacity of (100) and a current census of (34). LPAs conducted an overall inspection, which included, but was not limited to, the following:

Operation/Physical Plant: The facility is operating at a 3:1 client/staff ratio. The facility has an infection control and emergency disaster plan for review. Indoor passageways were kept free of obstruction. Activity rooms were maintained clean and odor free. Client bathrooms were maintained in a safe and sanitary condition. The hot water temperature tested at 105 degrees F. The facility is equipped with smoke/carbon monoxide alarms, fire extinguishers, telephone service, emergency food/water, and first aid kit with manual. An annual fire district inspection was conducted on January 9, 2026. Cleaning supplies and other dangerous items were kept locked. The facility has refrigerators and storage space for clients to store lunches. Snacks are provided by the facility.

Record Review: Four (4) staff records reviewed had First Aid/CPR certifications, criminal record clearances, job training, and health screenings. Four (4) client records reviewed had admissions agreements, needs and service plans, and medical assessments.

No deficiencies were cited during today’s visit. An exit interview was conducted were this report (LIC809) was discussed with Case Manager Mitchell and a copy provided at the conclusion of the visit.
