Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to conduct a required annual inspection of the facility. LPA identified self and discussed the purpose for the visit with facility representative, Rufina Laygo. The facility currently has no clients in care and vendor certification is pending with Inland Regional Center (IRC).
Physical Plant: All indoor and outdoor passageways are kept free of obstruction. There are no pools or other bodies of water located on the premises. The facility has sufficient indoor space and games for client activities. The facility's outdoor activity space is covered and enclosed with a self-latching gate. The facility is maintained at a comfortable temperature of 71 degrees Fahrenheit. Client (4) bedrooms were equipped with bed, chairs, nightstands, storage space and lighting. Client bathrooms were maintained clean and equipped with slip mats. The hot water temperature in clients bathrooms measured 120 degrees Fahrenheit. The facility is equipped with fully charged fire extinguishers, fire/carbon monoxide alarms, first aid supplies/manual, hallway night lights, bottled water and telephone service. The facility has posted in a common area: client personal rights, house rules, evacuation sketch and emergency telephone numbers.
Kitchen and Food Service: The facility's kitchen and three (3) dining areas are maintained clean. The facility is equipped with sufficient perishable and non-perishable supply of food. Sharps and cleaning agents are kept locked and secured.
Medication: The facility maintains a locked area where medications will be stored.
Record Review: The facility currently has no staff or clients for a file review.
No deficiencies were cited during today's visit. An exit interview was conducted where this report was discussed and copy provided to the facility representative at the conclusion of the visit.
