Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a joint Case Management preinspection Visit with fire inspector, Javier Gaona for change of ambulatory status to nonambulatory. LPA was greeted by Staff, Julia Broadway at the front door. LPA introduced self and stated purpose of the visit.
During today's visit, LPA discussed the changes that will be made to convert the two ambulatory bedrooms into nonambulatory approved. LPA did a health and safety check and conducted a tour.
No deficiencies were observed during this visit. An exit interview was conducted where this report was, reviewed, discussed and then provided to Julia Broadway.
