Facility Evaluation Report
On 7/16/2026 Licensing Program Analyst (LPA) LaVette Farow conducted an unannounced case management visit for Complaint number 56-AS-20250710131820 to gather signatures from the Administrator. LPA was greeted and explained the purpose of the visit to Administrator, Aveon Popoola. LPA Farlow conducted a tour and completed the report.
During today's visit, LPA met with Administrator to discuss the incident and the expectation regarding the needs of the clients and facility Program Statement. LPA completed a health and safety check.
At the conclusion of the visit no Deficiency was issued. An exit interview was conducted where this report LIC809, was discussed and then provided to Administrator, Aveon Popoola.
