The inspector’s account

Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced case management visit to follow up on an incident report sent to licensing dated on 11/13/25. LPA was greeted and explained the reason for the visit to Staff, Ausa Tuasina. Ausa phone called Administrator, Aveon Popoola and informed her about the purpose of the visit. Shortly after, Administrator Aveon Popoola and staff, Shanellreya Knowles arrived at the facility.

Per the incident report, a client called law enforcement because a staff pushed/bumped them and did not allow them to pass by. Record review revealed that the facility conducted an internal investigation and the staff was transferred to a different facility. During video footage review, it appeared that the staff attempted to redirect the client to prohibit them from walking on a wet surface for their safety. The client forced themself and passed through while ignoring instructions.

No deficiency was cited during this visit. An exit interview was conducted where this report LIC809 was discussed and provided to Administrator, Aveon Popoola.
