Licensing Program Analyst (LPA) Bernadette Allen an conducted an unannounced visit to amend documents signed on 9/28/2022 for the complaint investigation #56-AS-20220927151703. The house manager Shazet Knowles was informed of the purpose of the visit and signed the amended document.
An exit interview was conducted where this report was discussed, and a copy was provided to Shazet Knowles at the conclusion of the visit.
