The inspector’s account

Licensing Program Analysts (LPAs) Michelle Echeverria and Eldin Serrano arrived at the facility unannounced to conduct a post licensing visit. LPAs were greeted by Licensee, Joe Sims and granted entrance. LPAs introduced self and stated the purpose of the visit. LPAs observed that there are currently no clients in the facility and the facility is pending to get vendorized by Inland Regional Center. LPAs toured the facility with licensee.

The facility has 4 bedrooms, 2 bathrooms, a kitchen, dining area, living room, garage and backyard. LPAs conducted a general overall inspection, which included, but was not limited to, the following:

Physical Plant: The facility is maintained at a comfortable temperature of 76 degrees fahrenheit. LPAs inspected clients bedrooms; they are equipped with required furniture per regulations. LPAs observed that the facility did not have sufficient linens and comforters. Technical violation issued. LPAs inspected clients bathrooms; bathrooms were clean and appliances were operating appropriately. Water temperature in the bathroom tested at 108 degrees fahrenheit. The facility is equipped with operating fire extinguishers, smoke alarms and carbon monoxide alarms. Posters such as; the personal rights, visiting policy, and disaster plans were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept locked. There was a designated locked storage space for clients/staff files, first aid kit and medication. LPAs observed the fireplace with a broken glass cover. Technical violation issued. There are no pools, bodies of water, firearms or ammunition. LPAs observed the carpet dirty with multiple stains in each bedroom and the laminate floor border frame broken by the entrance and living room. LPAs observed the doorbell wires exposed. Technical violation issued. LPAs discovered that the facility was rented out to a staff. Technical violation issued. LPAs observed that the phone was not functioning because service was disconnected. Technical violation issued. Overall, the facility is not clean, in good repair, and operating in safe conditions for future clients in care.
