Facility Evaluation Report
On 7/2/25 at 2:00 pm Licensing Program Analyst (LPA) Noel Wolf Petersen, arrived unannounced to conduct an annual inspection, the LPA met with Licensee/administrators Digna and Albert Cabrera and explained the purpose of the visit. Cabrera's Guest Home is a ARF for with a capacity of 4 and a current census of 3. 2 are ambulatory, 1 is non-ambulatory.
The Physical plant was inspected, including but not limted to: the kitchen, client bedrooms, client bathrooms, common areas, the exterior, hallways, storage areas, and evacuation routes. The facility is clean and traffic areas are well lit and unobstructed. Toxics, sharps, and medications are stored locked and there is 2 days of perishable and 7 days of Non-perishable food supply. The bedrooms have required furniture and furnishings, encasements for the bedrooms were discussed by the LPA for beds with mattresses which were not wrapped in rubber, 1 of 4 beds. The client bathrooms hardware is in good repair, they are clear of toxic matierals and have trashcans with closeable lids. common areas furnishings are in good repair. the exterior screens are in good repair, it is generally clear of trip hazzards/debris and has adequte space for actiities. the storage areas are locked. evacuation routes and hallways are clear of obstruction and accessible to the non-ambulatory. Required postings including federal work rights, ombudsman, personal rights and facility sketch with evacuation route are present and of the correct dimension, the Fire extingusiher is dated 5/8/25 and the water temp is in required range of 105-120*F, the smoke can CO alarms are functional.
