On 7/23/26 at 3:00pm, Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced case management inspection to obtain documents requested by the department. LPA met with staff member Misi Qadroka and explained the purpose of todays visit.
LPA met with staff present, and requested the file for R1 (See confidential names list, LIC 811 dated 7/23/26).
LPA requested and obtained a copy of R1's admission agreement which was signed on 5/14/26.
Exit interview conducted and a copy of this report was left at the facility.
