Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding the previously discussed incident that occurred on January 28, 2025. LPA met with Administrator and explained the purpose of the visit.
Today's visit, LPA conducted a file review for S1. Additionally, LPA gathered a copy of S1's file along with staff training.
Administrator was informed this matter remains under review by the Department.
Exit interview conducted and a copy of report was provided.
